If you file as certified/approved kosher store or restaurant, you must fax us copy of approval certificate for your business that proves valid certification by the authorized rabbinical consulate. If you file as supermarket that sell kosher food product - your submitted application will be retrieved and processed. Application can be completed on-line. You can also choose to download a copy of the application available in PDF format. The application may be printed, completed and mailed to the address at the top of this document.
